Overview of Quadruple Marker Test

A quadruple marker test is done to determine whether a developing foetus is at risk for certain birth defects or not. This test is done between 16-20 weeks (second trimester) of pregnancy. Four markers evaluated in the quadruple marker test are-

  • Human chorionic gonadotropin (HCG), a hormone released by placental cell
  • Unconjugated estriol- produced by the foetus and placenta
  • Alpha-fetoprotein- produced by the foetus’s liver
  • Inhibin A- produced by the placenta

Why and When Is a Quadruple Marker Test Done?

A quadruple marker test is done during the second trimester of pregnancy to assess the risk for certain congenital birth defects in the developing foetus. A normal baby has 22 pairs plus one pair of sex chromosomes. In a trisomy, there is an extra chromosome present. This extra chromosome may be either at chromosome- number 21 (Downs’ Syndrome), number 18 (Edward’s syndrome) or number 13 (Patau’s syndrome).

The quadruple marker test also uses an NT scan which utilizes ultrasound technology, which together with the biochemical tests allows doctors to perform a risk assessment of chromosomal abnormalities such as Down syndrome that may exist in the developing fetus. 

Females that have a risk assessment of 1:250 of trisomy are considered to be screen-positive, with further confirmatory testing required. Risk assessment can be divided into low, intermediate and high risk for trisomy.
